If this is your first time recieving a treatment from me please take a moment to fill out a health history form . You may fill out the document in any of the following three methods: • Fill out the online dynamic pdf health history form (requires Adobe Acrobat Reader version 6.2 or higher).You may then print the form and bring it with you to your appointment or save the form and email the attached file to johntanner.rmt@gmail.com • Print the document, complete the information by hand and bring it with you to your first appointment • Fill out the health history form when you arrive at the clinic (please come an additional 10 minutes early if you choose this option.)
